Psychiatry: Site Evaluation

For my psych evaluation, I present two H&Ps and a journal article. For my first site evaluation, I presented a 33-year-old female with suicidal ideation, who was brought in by EMS after her husband found her on their rooftop. She was brought into CPEP and was given risperidone, and Trazodone to help her sleep. The patient was suffering from depression and adjustment disorder. She recently moved from Bangladesh to the United States with her husband and two-kids, but her husband is still talking to his ex-wife and is making her feel worthless. She was discharged within two days and was advised to follow up with the Adult Outpatient Psychiatry Department.

For my second site visit, I presented a patient who was brought in by EMS and NYPD but not under arrest due to his aggression at his group home who had a past medical history of Bipolar disorder, and marijuana use. Due to his aggression, it was likely patient had substance-induced mania.  Because the patient was agitated, and uncooperative he was given Haldol 10mg/ 5mg Ativan to sedate him. Professor St. Martin explained that sometimes marijuana is laced with phencyclidine, and is something you should always check when performing a urine tox.

Psychiatry is a complex field of medicine, very different than internal medicine. It was beneficial to discuss these cases with Dr. Saint Martin and receive feedback, as well as his accounts in working psych.
